Initial Phone Call or Email

We start with an initial phone call or email connection to assess your wants and needs.

Free Home Consultation

We’ll meet with your family to assess the client’s needs and wants. This will include an ANCHOR Plan of Action, a Home Safety Check and a Proposed Schedule for CARE. 

Develop Individualized Plan of Action (IPAc)

We’ll work with you to develop an Individualized Plan of Action for your loved one. Family members will be given the opportunity to follow the client’s Individualized Plan of Action through an online ‘Family Room’.

Caregiver Connection

We encourage family to participate and be involved in the process to ensure caregiver compatibility, scheduling and comfort with the new family dynamic.

Ongoing Support

ANCHOR is committed to assessing, re-assessing, and addressing changing roles and lifestyles. Our caregivers are given tools with which to manage care, recognize changes, and alert Project Specialists of any concerns. Clients become part of the ANCHOR family and are treated as such throughout their relationship with us.

Individualized Plan of Action

Ongoing Support

When placing a caregiver in someone’s home, communication is key.  Family members, caregivers, and professionals are each an integral link in making a successful transition.

Anyone may have difficulty living independently or could develop a medical issue that hinders the ability to cope alone, thereby producing a sense of loss.
